Smoking in United States of America

In the United States of America, smoking is allowed but tobacco products should follow some regulations. Under these regulations, health warnings on tobacco packaging including cigarettes are mandatory, the minimum legal age for their sales is 21 years, and there are no federal legal restrictions on smoking in public places. While tobacco advertising is regulated, not all forms of their advertising are banned. An excise tax of 32.16% with a total taxation rate of 37.36% is applied on cigarettes. Cigarettes can legally be sold with authorised documents. They may also be purchased through vending machines as well as through online channels. Importing cigarettes for trade is allowed with an appropriate import license. Despite these control measures, prevalence data show that smoking remains an important public health issue in the United States. There were 39.8 million current smokers in the country in 2024. This represents an overall adult smoking prevalence of 14.1%, with higher prevalence among men (17.1%) than among women (11.1%). In comparison, the overall current smoking prevalence was 12.3% in 2022. The adult daily smoking prevalence was recorded to be 10.01% in 2020. Smoking-related mortality also remains significant in the country. In 2021, smoking resulted in 360,370 deaths, including 216,986 male deaths and 143,383 female deaths. This shows that smoking accounted for 10.38% of all deaths in the United States in 2021, with a higher rate among men (11.9%) than among women (8.7%).

Top Misconceptions in Tobacco Harm Reduction – 2023

On August 15, Brian King, Director of the Food and Drug Administration (FDA) Center for Tobacco Products (CTP), acknowledged that there are opportunities for the CTP to educate consumers about the relative risks of tobacco products. By communicating that some tobacco products carry less risk with them than traditional cigarettes—such as electronic nicotine delivery systems (ENDS), heated tobacco products and oral tobacco products—the burden of smoking-related death and disease could be significantly impacted. [...]

Is it time to de-demonize this drug?

In the early 20th century, Americans demonized alcohol. In 1920, they codified that demonization in the 18th Amendment to the Constitution, prohibiting alcohol. During the roaring ’20s, in American homes and speakeasies all over the land, Americans thumbed their noses at that demonization. As did bootleggers, smugglers and organized crime bosses. In 1933, Americans decided to de-demonize alcohol with the 21st Amendment, making this psychoactive drug — alcohol — legal again.

FDA must close flavored vape loophole

Earlier in my career, I spent a fair amount of professional time working with companies on U.S. Food and Drug Administration compliance. While I worked on the business side not the government side, I recognized then and now how important it is for us to have an agency tasked with protecting consumers from dangerous products. When that agency is functioning properly, that is. Sadly, today’s FDA is falling down on the job when it comes to enforcing its own regulations regarding illicit flavored disposable products, especially products from China.

FDA’s Approaching Menthol Cigarette Ban Would Expand the Drug War

A national ban on menthol cigarettes is inching closer. This month, the United States Food and Drug Administration will publish final regulations on the matter, according to the agency’s Spring Regulatory Agenda. The FDA has been considering making the ban effective one-to-two years after publication. A public comment period last year saw 175,000 comments submitted on the proposed menthol cigarette ban, with another 71,000 for a similar restriction on cigars. Based on my review, a majority appear to be against the ban.

E-Cigarette Use on the Rise in US Adults With CVD

“E-cigarettes have been considered a safe means of smoking cessation for combustible cigarettes. However, the evidence of cardiovascular harm of e-cigarettes has been accumulating in recent years, so it is necessary to understand the proportion and trend of e-cigarette use in people with CVD,” said Shanjie Wang, MD, PhD (Second Affiliated Hospital of Harbin Medical University, China), who shares senior authorship on the paper with Yiying Zhang, MD, PhD (Jiamusi University, China).

Commentary: Dr. Matthew Holman — FDA Getting It Right on Youth Vaping

The message that all Americans need to hear from health experts is a simple one: If you don’t use tobacco products now, don’t start. If you smoke, quitting is the best choice. If you don’t quit, change to a smoke-free alternative. This message is supported by science, as evidenced by FDA’s authorization of nicotine products that, for adult smokers who completely switch to them, can significantly reduce their exposure to the harmful chemicals found in smoke or, for some products, reduce the risk of lung cancer and other smoking-related diseases.

Surge in Adult Vaping Underlines Our Harm Reduction Priorities

While youth vaping continues to dominate news headlines, adults who use vapes have been sidelined. Unfortunately, the Centers for Disease Control and Prevention (CDC) has been less than transparent when providing data on adult vaping in the United States. But recently, the agency did provide updated numbers, and these indicate that adult nicotine vaping is fast increasing.

A Comparison of US and UK Physicians Advice on Nicotine and Vaping

In 2020, an academic paper suggested that more than 80% of U.S. physicians mistakenly thought that nicotine was a carcinogen. The implication of this finding was that perhaps physicians thought vaping (and even nicotine-replacement therapy) to be almost as dangerous as smoking. But physicians are busy people and I suggest that some, maybe most, might have misunderstood the question in the survey and assumed the researchers were asking about smoking.

To test this hypothesis, I surveyed physicians to learn more about their actual knowledge and opinion. Additionally, the UK government is (...)
