Smoking in Italy
In Italy, smoking is legal but tobacco products are subject to national tobacco control regulations. Cigarettes are regulated as tobacco products and may be sold through retail outlets, online platforms, and vending machines. The importation of cigarettes for trade is allowed with a license. However, health warnings on packaging are required, the minimum legal age for purchase is 18 years, and there are no legal restrictions on smoking in public places. A comprehensive ban on all tobacco advertising is in place. Cigarettes are taxed at a total rate of 76.69%, with an excise component of 7.42%. Smoking remains a significant public health issue in Italy. In 2024, there were approximately 10.6 million current smokers, representing an adult smoking prevalence of 20.3%. In the same year, smoking prevalence was higher among males (23.8%) than among females (16.9%). In 2022, the adult smoking prevalence was reported to be 20.4%. In 2023, the adult daily tobacco smoking prevalence was reported to be 18.7%, with an estimated 12.6 million daily smokers. Mortality data show that in 2021, smoking caused 61,365 deaths in Italy, including 45,844 deaths among males and 15,521 deaths among females. Overall, tobacco smoking accounted for 8.78% of all deaths in the country in 2021. These figrures represent 13.47% of all male deaths and 4.33% of all female deaths, demonstrating the substantial health burden associated with cigarette smoking in Italy.

Italy Celebrates Huge Reductions in Excise Tax on E-Liquids
With votes by Italy’s League and Five Stars (Coalition), the consumption tax on e-liquids is reduced by a significant 90% on non-nicotine-containing liquids, and 80% on the nicotine containing types. With regards to online sales, e-cig manufacturers will also be able to sell e-liquids online after registering with the tax (customs) warehouse. Additionally, from now on, manufacturers must comply with the European Tobacco Directive (TPD), and can only sell nicotine liquids that contain a maximum nicotine concentration of 20 milligrams per milliliter.

ANPVU launches to give Italian vapers a consumer-led voice
A new consumer advocacy group has been launched in Italy to represent the country’s e-cigarette users. The National Association of United Vapers - Associazione Nazionale per i Vapers Uniti in Italian – says its primary objective is to promote harm reduction by example, through introducing smokers to the new generation of safer nicotine products.

E-cigarette rules should be relaxed to encourage smokers to switch, MPs told
E-cigarette regulations should be relaxed to make vaping more attractive to smokers of traditional cigarettes, expert witnesses have told a House of Commons Science and Technology Select Committee hearing.
Riccardo Polosa, professor of internal medicine at the University of Catania, in Italy, told MPs that selling larger refill containers for e-cigarettes might persuade tobacco smokers to make the switch to vaping.
